Principle The Siemens ADVIA 1800 assay is an enzymatic creatininase method. The calibration is traceable to SRM967 and correlated with the NIST LC-IDMS method.
Interpretation Serum creatinine is not a sensitive indicator of early renal disease.

1. Creatinine elevated: impaired renal (glomerular) function, pre- and post-renal azotemia, certain muscle diseases (polymyositis, muscular dystrophy), muscle crush, intramuscular injections, exercise, and excessive growth hormone. Ingestion of creatinine (roast meat).

2. A 10:1 ratio of BUN:creatinine is seen in excess intake of protein, blood in the small bowel, excess tissue breakdown (cachexia, burns, high fever, corticosteroid therapy), urinary tract obstruction (post-renal), CHF (dehydration, shock), and with excessive urinary reabsorption (ureterocolostomy). A ratio of less than 10:1, BUN:creatinine, is seen in low protein intake, repeated dialysis, severe diarrhea or vomiting, and hepatic insufficiency.

Interferences Blood samples from some patients with disease states where significantly elevated immunoglobulin levels occur may have falsely elevated results when using this assay.
Additional information INDICATIONS: Assessment of glomerular function (in conjunction with BUN) and severe muscular function abnormalities.
