In House Availability Monday - Friday. Immunodiffusion test results are available 2-7 days after blood is received in laboratory.
Principle Patient serum is tested for both "Early" (IgM) and "Late" (IgG) antibodies to coccidioides using an immunodiffusion procedure. Patient serum and known antigen are allowed to diffuse toward one another in an agarose medium and a visible line of precipitation forms if antibody directed toward coccidioides is present in the patient's serum.
Interpretation Presence of "Early" antibodies to coccidioides indicates recent, active infection. These IgM antibodies can be detected by immunodiffusion (ID) within one week after onset in 90% of patients with primary non-disseminated disease. The test for IgM will revert to negative within a few months, however these antibodies may reappear after rupture of a cavity or with systemic dissemination.

IgG antibodies directed against "Late" (or CF) antigen; indicate active or recent past (up to 1 year) infection. The ID test for IgG is usually positive within 4 weeks after infection and remains positive throughout the clinically active disease.

Demonstration of precipitin bands denotes recent, active, or inactive infection.

Note: IgM or IgG may not be detectable in patients with AIDS.
