Principle | Hemoglobins are separated and quantitated by an ion-exchange high performance liquid chromatography method (IE-HPLC, VARIANT II system, Bio-Rad). The method is based on electrical charge differences between hemoglobin molecules and the stationary phase in a chromatography column, resulting in distinct retention times for hemoglobin variants as they pass through the column. | ||||
Interpretation | QUANTITATIVE RESULTS, INCLUDING HEMOGLOBIN A2, F: * Hemoglobins A2, F, and S and C, if present, will be quantitated and reported. If MCV is below the reference range (< 80 fL), ZNPP (zinc protoporphyrin) will automatically be performed to evaluate for iron deficiency; the results will be reported. * For quantitative A2, F order "Hemoglobinopathy Evaluation" INTERPRETATION: For interpretation, consult the Laboratory Medicine Resident in Hematology (x65689; pager: 415-443-0179; pager, after hours and on weekends: 415-443-6969). |

Additional information | Include clinical history and order CBC. Both are required for interpretation. If no CBC is ordered, the lab will add this test. CONFIRMATION OF ABNORMAL HEMOGLOBINS: If there is no prior record of an abnormal hemoglobin, the specimen will be sent out to a sendout lab for confirmation. Other confirmatory tests which may be performed include sickle cell solubility test and hemoglobin H preparation. |
