The UCSF Clinical Laboratories utilizes several outside reference laboratories to perform laboratory testing that is not performed in-house. A minority of the send out laboratories request information in addition to the information requested on the requisition. The list below indicates additional information that must accompany the specimen. Please fill out a form below if you are requesting the indicated test.
