The UCSF Clinical Laboratories have limited resources to provide specialized report requests for quality improvement purposes. Additionally, there are some limitations: results are purged after 3 years, so this data is no longer available in the laboratory information system. Results are archived after a year, which increases the difficulty in running reports that include data over a year old: this requires running a separate report to capture this data.

Due to UCSF Policy, the Clinical Labs cannot respond to requests for data for research purposes directly. Please refer to the HIMS Data Request web page and the UCSF Medical Center Policy: Control of Access to and Release of Information from UCSF Medical Center Information Systems for Research Purposes Policy #5.01.06

The UCSF Clinical Laboratory Report Request form is available below for non-research, quality improvement purposes. A fee of $100/hr with a minimum of 2 hours will be charged for reports. The filled out request form should be sent electronically to Enrique.Terrazas@ucsf.edu or hard copy to Enrique Terrazas at UCSF Box 0451.
