The UCSF Clinical Laboratory manual lists tests that are performed by the UCSF Clinical laboratories as well as those that are available as send-out tests. Tests that are not listed in the laboratory manual may be available with approval of a laboratory resident or attending for send-out to CLIA certified laboratories on a case-by-case basis.

Requests to add a test or tests to the Laboratory manual may be made by completing the form below and sending it to Mr. John Chapman at Box 0134. The Clinical Laboratory Utilization Subcommittee reviews such requests and will determine if the requested test or tests should be added, added with service restrictions or not added to the manual.
