| Collection Instructions | The physician must complete the CDC DASH form. Click here for form together with a routine laboratory requisition requesting this test. The ordering physician is also required to notify SFDPH Communicable Disease Control by phone (415-554-2830), fax the completed DASH form to SFDPH and notify the California DPH Microbial Diseases laboratory at (510-412-3700). Please indicate that the notifications were made on the laboratory requisition. |

| Processing notes | Refrigerate sample. If a CDC DASH from is not rec'd with the sample contact the ordering physician to provide this. The form is available via a link from the lab manual page for the test. If the requisition and/or DASH form does not indicate that SFDPH and the California DPH Microbial Diseases Laboratory were contacted by the ordering physician, attempt to comntact that individual and either confirm they have made the notifications or that they need to do so.
